Forced-Choice Reinforcement Survey K-12

This is a useful instrument that ranks student preferences into categories as follows: adult approval, competitive approval, peer approval, independent rewards, and consumable rewards.  For best results, complete this interview-style.

 

Reinforcement Survey ES/MS/HS This includes simple preference ratings on a scale of 1-2-3 for food, activities, social, and tangible reinforcers (3 pages)

 

35 Ideas for Reinforcer Menus ES/MS/HS  These are good ideas for getting started in tier 2 or tier 3 however they are not separated by functions of behavior, which would be most valuable.

 

Reinforcement Survey with Open-Ended Questions for ES/MS/HS

This is very simple and possibly a good place to start in tier 2 or tier 3.

 

Reinforcement Interview_In-Depth (but EASY) Functional Assessment Survey ES/MS/HS 

This is more comprehensive BUT it is EASY to administer.  There are 5 sections.  Section A has 12 items for general info using a 3-point scale; Section B has 5 open-ended questions related to target behaviors; Section C is a Preference Rating of academic subjects; Section D asks for Likes and Dislikes for each class; Section E asks student to identify favorites across categories including edibles, tangibles, academic reinforcers, social reinforcers, activity reinforcers, and recreation/leisure reinforcers.  This is worth your time.

 

ZZ Potential Reinforcer Interview for ES and low incidence

This can be administered individually or in small groups.  There are 40 forced-choice items that result in a rank-ordered hierarchy of potential reinforcers in the following categories: social, privileges, edibles, tangibles, and activities.  There are 14 open-ended questions about student interests.  The instrument includes a reinforcement menu.  This is most appropriate for ES and low incidence populations.
